The Phoenix Union High School District offers a generous and comprehensive benefits package to eligible employees and their dependents. The District provides Health (Middle Option), Mid-Term Disability and Life Insurance on employees at no cost.
There is also additional coverage that you may purchase, such as dependent health insurance, dental, short-term disability and voluntary life insurance.

contracted employee working a minimum of 24 hours per week, or
Metro Tech employee working a minimum of 20 hours per week.
Your legal dependents include your:
lawful spouse; and
dependent, biological, step or legally adopted child(ren) who are under age 26, whether married or unmarried.
If your contract starts between the 1st and the 15th of the month, your insurance will be effective the first day of the following month. If your contract starts between the 16th and the end of the month, your insurance will be effective the first day of the second following month.
Open Enrollment Changes
All changes made during Open Enrollment will become effective on July 1st.
Qualifying Event Changes
Medical and dental premiums are deducted from your paycheck on a pre-tax basis—giving you the advantage of not paying tax on your healthcare premium dollars (unless you let us know that you want to pay premiums with after tax dollars by completing the Flex Plan Options form).With the pre-tax feature, IRS rules require that you cannot change your elections until the next Open Enrollment period unless you have a qualified change in status that affects your benefits, such as a birth, adoption, dependent losing dependent status, marriage, divorce or death. Note: You must notify the Plan in writing within 30 days of a qualifying change in status.Changes made due to a qualifying event will become effective immediately. Premium changes, if applicable, will be effective the first day of the following month.
Coverage for eligible dependents who are included on an employee's enrollment form becomes effective the same date as the employee's coverage.
Termination of Coverage
If not a returning employee, benefits will terminate on the last day of the month in which your contracted employment ceases.
The District's medical insurance provider is Blue Cross Blue Shield of Arizona (BCBSAZ).
The District’s medical insurance gives you three options:

Low Option Plan
Understanding the differences between the medical plan options can help you decide which plan is best for you and your dependents. All plan options are preferred provider organization (PPO) plans accessing the BCBSAZ network of providers. With PPO plans, you can see any provider you wish, but you get greater cost savings by using in-network PPO physicians, hospitals, labs and urgent care centers. This is important to consider since you share in the cost of care in the form of co-pays and coinsurance.

The District offers two dental plan options: TDA Prepaid Plan or Delta Dental Indemnity Plan. Dental coverage is independent of medical coverage; you can elect to participate in a dental plan even if you waive medical coverage. Note: Participation in dental insurance is voluntary; the District does not pay any employee or dependent premiums
Total Dental Administrators (TDA)
TDA has an extensive network of providers throughout Arizona. The Prepaid Plan provides coverage for services and treatment for diagnostic, preventive, restorative, and routine oral surgery. Services are paid according to the TDA fee schedule and you must utilize a TDA provider. For more detailed information on the fee schedule and to locate a TDA Provider, please refer to the TDA Summary of Benefits and Locating a Provider Sheet via the Insurance Forms menu to the left.
Delta Dental’s Indemnity Plan
The Indemnity Plan utilizes the Delta Dental network of dentists, or you can see any dentist and submit a claim form for reimbursement. This plan covers preventive dental services such as oral cleanings, examinations, and X-rays. Preventive care is covered at 100% with no deductible. Basic services are covered at 80% and major services are covered at 50% after you have been on the plan for 6 months (continuous). Orthodontic services are available for children, 8 to 18 years old, after 12 months (continuous) on the plan. Life & Accidental Death and Dismemberment (AD&D) Insurance
Basic life insurance coverage is provided to all eligible employees. This coverage is automatic and paid 100% by the District. You receive one times your base annual earnings with a minimum benefit of $25,000 (maximum benefit restrictions may apply).
AD&D coverage is also provided to all eligible employees. This coverage is automatic and paid 100% by the District. You receive one times your base annual earnings with a minimum benefit of $25,000 (maximum benefit restrictions may apply).
Voluntary Life Insurance
In addition to your District-paid life insurance, you have the option to purchase additional (voluntary) life insurance, underwritten by Standard Insurance Company. For more information, please refer to the Standard documents via the Insurance Forms menu to the left.
You may apply for voluntary life insurance coverage for you and your spouse in increments of $10,000, to a maximum of $300,000. Note: Your spouse cannot have higher coverage than you. Also, employees cannot have personal coverage and be covered as a spouse through another District employee.
You may also apply for the following voluntary life insurance coverage for your dependent child(ren):
Age 15 days to 6 months: $1,000
Age 6 months to age 26: $2,500, $5,000, $7,500 or $10,000.

Long-term disability insurance provides a weekly income of up to 66 2/3% of your base monthly pay. Coverage is provided through the Arizona State Retirement System (ASRS) and is available after six months of disability and after you have exhausted any remaining sick leave.

Mid-term disability insurance provides a weekly income of up to 66 2/3% of your base weekly pay (maximum of $2,500) for a maximum of 90 calendar days if you cannot work due to a non-occupational, accidental injury or illness (including pregnancy). Benefits are available after a 90-day waiting period and after you have exhausted any remaining sick leave.
This coverage is provided automatically for employees and paid for by the District.
Short-term disability (STD) insurance provides you with income of up to two-thirds of your base monthly salary if you are totally disabled and unable to work. Benefits start on the sixth day of disability and are payable for up to 90 calendar days.This benefit has a workers’ compensation exclusion.Participation in Short Term Disability is voluntary; the District does not contribute towards this premium.
Flexible Spending Accounts
IRS laws allow eligible employees to make pre-tax contributions to flexible spending accounts to pay for out-of-pocket health care expenses and/or employment related dependent/elderly day care expenses.
Health Care Account
Enrolled employees can contribute up to $2,500 on a pre-tax basis every fiscal year and be reimbursed for medical, dental, prescriptions, and vision expenses not paid by insurance. Examples of eligible expenses may include co-pays, deductibles, orthodontia, eyeglasses, and certain over-the-counter medications. A debit card is provided for your convenience.
Dependent Care Account
Enrolled employees can contribute up to $5,000 on a pre-tax basis every fiscal year and be reimbursed for eligible daycare expenses. Eligible expenses may include qualified daycare centers for children and adults, babysitters, pre-school, and general purpose day camps while you work.

Employees are automatically covered by Worker's Compensation insurance through the district's current worker's compensation carrier,The Alliance. If you are injured on the job, immediately report your injury to your supervisor; your supervisor will guide you from there.
The District provides Bus/Light Rail Passes for all employees for use during their contracted work schedule. Please contact the HR Office @ 602-764-1100 for more details.
All insurance benefits via the PUHSD will terminate on the last day of the month in which your contracted employment ends. You will be offered COBRA continuation coverage whereby you will be eligible to keep your medical and/or dental insurance for an additional 18 months; the PUHSD does not contribute towards COBRA. You can also convert your life insurance to an individual policy; if interested in conversion, please contact the Benefits Department @ 602-764-1538.
Unused leave will be paid off @ 45% and unused vacation (12-month employees only) will be paid off at 100%. Note: Payments over $5000 will be subject to the Special Pay Plan.
